Carl D. Forman, 73, of Evanston, died Saturday, Oct. 30. He was born on Sept. 24, 1948, in Evanston, the son of Dean and Laura Limb Forman. 

He was a proud American who enlisted in the United States Navy at the age of 17, where he served in Vietnam on the USS Providence.

Carl was an avid hunter and outdoorsman and enjoyed being a handyman. Many know him as “Mr. C.”

Survivors include his wife, Eleanor, of Evanston; four daughters from a previous marriage: Kristina Ruiz (Rodney) of Evanston, Stephanie Miller (Kyle) of Carbondale, Pennsylvania, Amanda Schuback of Dunmore, Pennsylvania, and Loralee Matthews (Ryan) of Manteca, California; brother, Jim Forman of Silverton, Oregon; Norma Merrill of Evanston; 11 grandchildren; 3 great-grandchildren; and numerous nieces and nephews.

He was preceded in death by his parents and sister, DeAnn Fowler.

A memorial service and potluck luncheon will be held from 1-5 p.m. on Sunday, Nov. 14 at the Machine Shop in Evanston. Please stop by to pay respects, share memories and help celebrate Carl’s life.
